Many employers sponsor matching gift programs and will match any charitable contributions made by their employees.

If your company is eligible, request a matching gift form from your employer, and send it completed and signed with your gift. We will do the rest. The impact of your gift to this organization may be doubled or possibly tripled! Some companies match gifts made by retirees and/or spouses.
If you do not find your employer listed, please contact your human resources department to see if you can establish a matching gifts program with the ALS Association Greater Philadelphia Chapter.
How Do I Apply for a Matching Gift?
- Obtain a matching gift form and program guidelines from your Community Initiatives or Human Resources Department. This information may also be available on your company Intranet site.
- Complete the Employee section of the form and send it to the ALS Association, Greater Philadelphia Chapter along with your donation or, if you’ve made your donation online, simply complete the form and mail it to:

- We will complete the form and forward it to your employer’s matching gifts office for processing. The company will send its matching donation directly to the ALS Association, Greater Philadelphia Chapter.
The ALS Association is a 501(c)3 nonprofit organization and donations are tax deductible to the full extent of the law. The ALS Association’s federal tax ID number is 23-2387205.
Some companies disburse matching gifts on a quarterly or yearly schedule. To reduce administrative expenses, we will not forward a notice to employees when matching contributions are received. If you ever have a question about your company’s matching donation, please contact your matching gifts office or human resources department.
